Old Coulsdon Medical Practice has changed its appointment booking system. Instructions on how to use the new system can be found below. If you do not have a smartphone, computer or similar device you can still call the practice who will complete the forms for you.
Please Note: you can no longer book an appointment directly by phone.

Sign into the NHS App on your phone or the OCMP Website on your PC/Tablet or Phone

If using NHS App

Once you access the app click on View all which is situated opposite the Services under your NHS number.  This takes you to Your GP services.

Select Contact your GP about a health problem and the following screen will appear – click on Continue.

Next you will be presented with a screen which tells you the times the service operates (Mon-Fri 7.30am – 5.00pm) and more about the service.  You will need to confirm that it is not an emergency and advised to call 999 if it is.

Next you will see a Medical request form where you will need to complete a series of questions about your medical problem.

Please describe the medical problem (ie I have back pain) and you can attach a photo if you wish to.

How long has this been going on for?  Is it getting better or worse?

(ie 2 weeks and it is getting much worse)

Have you tried anything to help?

(ie I have tried ibuprofen gel but it isn’t helping)

Is there anything  you are particularly worried about? (optional)

(ie I’m very worried about it affecting my work.

How would you like us to help?

(ie I would like to get help with the pain and I may need a certificate for work)

Please tell us the best times to contact you (optional).  We can’t guarantee a time and will only contact you during practice opening hours.

(ie I can be available most of the time but please ensure to call my mobile phone 0777 xxxxxx

Click on Continue

You will be asked who the request is for, Myself or Someone Else

Click on Continue

You will then see Your details  (Name, Date of Birth, Postcode, Phone Number) and you will be given an option of how you want to be contacted.  Phone is preferred but you can select Text message.

Then you will be asked if you have access to the phone number you have provided right now?

When you confirm and continue you will be sent a verification code to your mobile which you need to type in.

If using OCMP Website

www.oldcoulsdonmedicalprac.co.uk  simply select Appointments and then select Contact Us Online.

The process is the same as with the NHS App but your personal details will not automatically be completed (as no connection to your NHS records) and you will not be sent a verification code.

NOTE:  if you do not have access to a smartphone, tablet or computer you can still phone the surgery for an appointment.  But, appointments can no longer be requested or booked immediately by phone, the person who answers will need to complete the same request form for you over the phone and this will be assessed in exactly the same way as those submitting by app or online.
